Part A – Create Form in Gmail

Go to Gmail Go to Gmail at www.gmail.comand “Sign in”using your Gmail account

Enter Google Document Click “Document” on the top after enter Gmail

Create Google Form You will see this site after clicking “Documents”, click “Create new” on the left hand side

Create Google Form A slide bar is shown, select “Form”

Creating Google Form A new page is shown, you can now edit your form Title here will become the name of the file and Title of the Form Explanation of the file (optional) Yellow Zone means you are editing this Question

Type of Question There 7 types of Question: Text Paragraph text Multiple choice Checkboxes Choose from a list Scale, Grid

Type of Question: Text Used if short answer is required e.g. Full name of the user Type your Question here, remember to number if Help text in this Question

Type of Question: Paragraph text Used if long answer is required e.g. Comment to the course structure Type your Question here, remember to number if Help text in this Question

Type of Question: Multiple choice Used if one answer is required, given a number of choices Type your Question here, remember to number if Help text in this Question Click “x”, will delete one Option, Here “Option 3” will be deleted Click “add “Other”” will add Option “Other” with text box for input: Click the text box, “Option 5” will be shown

Type of Question: Checkboxes Used if more than one answers are required, given a number of choices Type your Question here, remember to number if Help text in this Question Click “x”, will delete one Option, Here “Option 3” will be deleted Click “add “Other”” will add Option “Other” with text box for input: Click the text box, “Option 5” will be shown

Type of Question: Choose from a list Used if one answer is required, given a large number of choices, e.g. asking for home country Type your Question here, remember to number if Help text in this Question Click “x”, will delete one Option, Here “Option 3” will be deleted Click the text box, “Option 5” will be shown

Type of Question: Choose from a list Used if rating is needed E.g. Rating of performance Type your Question here, remember to number if Help text in this Question Rating from 1 to 5 Example on NEXT page Label shown on top of 1 and 5

Type of Question: Grid Used if a number of Questions having same answers. E.g. Performance of Lecturer Go to next page to see an example

Part C – Editing Google Form

Add Section Header Section Header is the Section title of your form

Add items Click “Add item”, then choose one of the type of Question

Edit Added item Moving cursor here, background color of this question will change Click this Icon or Double Click will edit this Question

Editing Google Form Finish editing this Question by clicking this icon Duplicate this Question by clicking this icon Delete this Question by clicking this rubbish bin Enable this will require user to answer this question before submit the form Finish Editing this Question, Click “Done”

Moving Question Drag and Drop can move Question 4 to Question 3

Usage of Page break Page break can be used when there is Multiple Choice type Question

Usage of Page break Page break can be used when there is Multiple Choice type Question If you want to jump from Q1 to Q4, use page break Enable this items, following items will be shown You can choose which page to jump to if user choose this option

Part D – Finishing Google Form

Share Form to Others Back to Document in Gmail, new item is added

Share Form to Others Choose one that is suitable for you Tick this one, color of column changed

More Action Provide Link for you to embed this form Reply letter to the finish user
