The primary purpose of this survey is to gauge the audience's experience with Google Docs and Forms & Spreadsheets in particular.  It will help the instructor with the pace of the instruction. A secondary purpose is to serve as an example of all seven types of items possible on a Google Form and also how one can use branching on a Google Form.
