Hello and Welcome to the IGYM Fit|Life Preliminary Questionnaire. This questionnaire was designed for us to get a general but accurate snapshot of your health and wellness goals. You will notice that our questions do not only cover physicality, but also mentality. This is intentional. We define true health as being sound in both body and mind. This survey covers both bases.
